Carol Louise Smith (nee Hennessey) was born on January 24, 1944 in Prince Edward County, Ontario to parents Allan and Gladys (Tuttle) Hennessey. She passed away on April 21, 2018 at the Yorkton & District Nursing Home at the age of 74 Years. She was predeceased by her parents and son Dean Leavitt. She leaves to mourn her son Chris; grandchildren Beth, Joshua and Jenelle, five siblings and numerous nieces and nephews.
